Worker Failed while applying 17851150

Hello All,

I need some help troubleshooting this issue for my customer:

Ct is applying CPU patch in our env (11.5.10.2)

Patch Number : 18122009

Pre-Req Patch : 17851150

While applying the Patch 17851150 , A worker got failed and they were able to get it restarted the worker via adctrl and again the worker got failed out .

The log indicates the following:

TTENTION: All workers either have failed or are waiting:
  FAILED: file AFPFPROS.pls on worker  1.
ATTENTION: Please fix the above failed worker(s) so the manager can continue.
Current time is: Fri Sep 12 2014 12:04:05
    Fixed: file AFPFPROS.pls on worker  1 for product fnd username APPLSYS.
