Why does a promotion modifier get applied in order management orders and not on iStore orders?

Tried to create a promotion modifier of 12+2.  Upon testing in creating sales order using order management, a new line for bonus item of 2 units is created for a 12 units order.  However, this modifier is not getting applied in iStore orders.  What could possibly be missing?
