We are not able to receive lot controlled items

We are able to receive all items other than lot controlled items. When we go to receiving screen - after entering all the details we try to save and the form generates new receipt number however it does not save the transaction.  We have verified all the setups and tried with multiple items in different organizations, still no clue.
