Network Test form on 12.1.3?

I have seen references in various documents that describe a "Network Test" form, whereby you can measure various metrics regarding performance of the network that EBS is utilizing.  I am on 12.1.3 and I cannot seem to find this form anywhere?  Anyone know the form name on 12.1.3?
