planning Form do not show dimension memebers

Hi Experts,

while creating planning form, entity dimension shows only the dimension name and none of the members, although if we went to manage Manage-->Dimensions we can see the full hierarchy. All old forms are now not working saying that not all dimensions are represented on the form and when we check we see Entity dimension has no selected members. Please advice what should we look for to identify the issue.
