Receiving SBL-EAI-04381 when upserting through webservice after upgrade

I'm working on upgrading our system from 8.1.1.1 -> 8.2.2.4.11 and have not been able to work out this error with our webservice on what should be an upsert:

Error invoking service 'UCM Transaction Manager', method 'SOAPExecute' at step 'Transaction Manager'.(SBL-BPR-00162)

CIF Transaction Mgr service failed to process the record with SDH Row Id=1-5ARFEG. For instance of Integration Component 'UCM HE Constituent Address', using user key '[HE Address Type] = "Home" AND [Street Address] = "2303 Radio Station Blvd" AND [Effective Start Date] = "09/29/2014 00:00:00"', a record with identical values already exists in the Siebel database.

Please ensure that the field values in the input message are unique.(SBL-EAI-04381)(SBL-IAI-00512)
