RMAN Incremental Backup Fails> ORA-19504: failed to create file . . .

Hello, have been running successfully for months then in past 2 weeks getting errors below.  ADG environment and am backing up Primary database.  If I rename the file that already exists and re-run backup it fails on yet another duplicate name.  I have even deleted all backups and re-run, it is ok for awhile then I get these errors again.  Any help appreciated!
