Primavera CM 13.1 - How to edit/add logo on Reports and Forms?

Thanks to all in advance for your help -

We recently performed a reinstall of Primavera CM 13.1 (no SP) for one of our clients. The reinstall worked and they can access their project data and forms, however after the reinstall the company logo that displayed on their forms and reports is now a multi-colored blob in the logo frame. Can anyone tell me how to edit forms to repair/replace that logo on their forms? Needless to say, my experience with CM is somewhat limited.
