How to personalize Contact Center "Quick Task"

We are using Contact Center which auto launches "Quick Task" based on SR and Problem Code combinations.

Planning on personalizing the "Quick Task" form. When on "Quick Task" clicking on Personalize is showing the personalize option for CSCCCCRC (Contact Center) form.

Is CSCCCCRC the appropriate form to personalize "Quick Task"? If not which is the correct form for "Quick Task"
