"Failed to create new journal" error message

Hi. We have just upgraded to HFM version 11.1.2.3.500. Periodically when users create a journal they recieve the message "Failed to create a new journal". However although the message is displayed the journal does get created.

Anyone else experienced this issue? I have an SR opened with Oracle but no resolution so far.  The issue is no consistent. It happens intermittently.
