12.1.3 - SR Task >Task Note freeze

I have a user who when they are in certain SR>Task Tab> and they click on Task Notes the Note form opens but the system just spins.  It is hanging and the system never responds. 

I pull up the SR>Task>Task Notes, and it opens right up reflecting multiple notes.  I logged into our test system, recently cloned, as the user and it opens right up.

I had the user clear IE cache and Java cache, same issue.

What else should I be looking at to resolve issue for this one user?

Ideas?
