iStore review and place order page JSP customization

I am trying to customize an istore jsp page.The requirement is to add a new date column with calendar icon to standard review and place order jsp page .

Please let me know steps  how to add new date column with calendar icon to jsp page (Attached to template)? Thanks in advance. Its very urgent.
