How to change out Enterprise Server

Hi, I am using the document Doc ID 1071557.1, how to change out a Windows Enterprise Server and have some confusion in following this document.

Step-1. Verify MTR (our production server is E812/898 on Windows 2003). I want a new Ent 9.1 server on Win 2012. Is this MTR for 9.1 on Win 2012?

Step-2: Install the Host Code on the New Ent server (should I install the new 9.1 host code as this is what I want. But copy the E812/898 server into 9.1 server).

Will the folder structure be like

\JDEdwardsPPack\E910\PD910

\JDEdwardsPPack\E910\PD812 (copied from E812). Will this replace PD910 or copy in parallel to PD910 and PD812.
