PSUs Hang During "Installing TFA" Step

Hi

This is a three-node 11.2.0.4 cluster on rhel5 x86_64.  This cluster in particular really drags or hangs outright when the PSU reaches the "Installing Trace File Analyzer" step.  This morning while applying the Oct2014 PSU  to node 1, it stalled for more than 30 minutes at "Executing cmd: /u01/grid/11.2.0.4/crs/install/tfa_setup.sh -silent -crshome /u01/grid/11.2.0.4"     With the PSU process still going, I ran /etc/init.d/init.tfa stop (as root).  It reported TFA as stopped, and the PSU completed.  TFA was not restarted, so I started it.

Before starting opatch auto on node 2, as root, I tried command /etc/init.d/init.tfa stop.  This command stalled; I could not shut down TFA gracefully with this or tfactl stop (again, as root).  I started opatch auto for the PSU, then I had to kill the tfa processes at the command line when the PSU hit the tfa_setup.sh script.  Opatch auto on node 3 was slow, but it made it past the TFA step without me resorting to killing processes.  A few questions:
