How to do Import Items from Excel by using APC?

I have downloaded the WEBADI.xls file on to my system and populated all the required information. Updated the Transaction type to 'CREATE' as i need to create item at the master organization. But everytime when i upload the file, the items never gets imported.

This is the first time i am using this utility. Can you please share any related documentation on this?

All the time i upload, i get the following details in the logfile as below.
