Approval Routes set up with Approvals and notifications for Address Book in World A9.3

Hi

Can someone who is using the address book approval process, enlighten me on how do I set up multiple levels of Approvals and notifications. I have done the set up and the emails do not flow to the notification group for some reason that and the level of the approval and notification is the same and it could be different. An example of what we trying to achieve has been attached. Note the Approver is getting the mail to tell him to aprrove, no notification mail is sent.
