Forms applet not opening in R12.2

Hi Guys,

Facing one weird problem. In my R12.2 instance after clicking on any forms link, it downloads jar files etc and shows "applet started" in the end of java console output but Forms don't get launched in reality. I tried launching the form directly by following url  http://<hostname>.<domain>:<port>/forms/frmservlet but it says , "url validation failed,The error could have been caused through the use of the browser's navigation buttons" .

There are no errors/logs in the forms_server1.log or access.log.  This behavior is uniform across IE8(64-bit), Mozilla 33.1, Chrome 38.0.21 . OS is Windows 7-64 bit SP1 and I have already followed the note
