What transactions in a WMS Org can cause Cost Group commingling?

What transactions in a WMS Org can cause Commingling of Cost Groups.

1. Simple CG structure - keep Raw Materials and FG in two cost groups.

2. No Cost Group Assignment rules / strategies defined.

Yet we have commingled inventory for e.g. in FG we have the same item belonging to both CGs in a specific locator.

We are on 12.1.3 - what in your experience will cause commingling?

Once again, question is not about when oracle pops up commingling warning and stops you from doing your transaction - we are past that and already inventory is commingled.
