How can get 1 additional address line on the standard 1099's?

If Suppliers that have 4 address lines, how to add the line 4?


FOR EXAMPLE:
Assemblyman John Doe
4th District
123 Main Street
San Diego, CA 99999-0000

Need a 4th line,Otherwise get 4th District 123 Main Street and that is undeliverable
