R12 : RUP7 patch 18004477 fails

hello,

tried to install patch with adpatch utilitybut load of software stalls with following:

Reading jobs from FND_INSTALL_PROCESSES table ...

   FAILED: file perrec.odf   on worker  2 for product per username HR.
   FAILED: file pervac.odf   on worker  4 for product per username HR.

So, I ran 'adctrl' and chose 'Show worker status'

Then chose 'restart' but did not fix the problem.

There does not appear to be any available log.
