Create a New Activity (e.g., appointment) link

While I can create a link in a report to open contact form...e.g.,   @[html]&lt;a href="/OnDemand/user/ContactDetail?OMTGT=ContactDetailForm&ContactDetailForm.Id="@target="_blank"&gt;"Contact Details"</a>"

Can anyone share the syntax for a url, that when clicked in a report, would open the form to create a new activity for a contact?
