11.5.10.2 : Applying baseline patch failed: ORA-06512: at "APPS.FND_FLEX_LOADER_APIS"

hello,

We are applying baseline patches for upgrade from 11i to 12.2.4. This is our 2nd iteration. During the 1st iteration we didn't face the following issues, but we are getting it in the 2nd iteration .

Note the patches are applied after merging.
The patches that are merged here : 4111404, 6617445, 4455179, 4246648, 4688396, 6340621, 5153056, 6444221, 4435838, 5199025, 5215447, 5765223, 6010109, 4508966, 8332175, 6163754, 9760758

Uploading from the data file /env200/oraapp/Norashr/devappl/bis/11.5.0/patch/115/import/US/bisalrp.ldt
Altering database NLS_LANGUAGE environment to AMERICAN
Dump from LCT/LDT files (/env200/oraapp/Norashr/devappl/fnd/11.5.0/patch/115/import/afcpprog.lct(115.40), /env200/oraapp/Norashr/devappl/bis/11.5.0/patch/115/import/US/bisalrp.ldt) to stage tables
Dump LCT file /env200/oraapp/Norashr/devappl/fnd/11.5.0/patch/115/import/afcpprog.lct(115.40) into FND_SEED_STAGE_CONFIG
