PO are not getting revised

We are using 12.1.3. First time PO is created and approved with rev 0. Then we again updated the PO price. Then it created rev1. Now third time we are trying to update the PO price and posmise date . Status is changing to required re approval but approve button is disabled also revision is not changed from 1 to 2.

Did you guys face this issue
