Attempting to upgrade EBS 11.5.10.2 to 12.2.4 using existing database

I am working from document 1617461.1 to apply the latest AD and TXK updates. However, I cannot get  adop to run successfully.  Per the document, I executed:

$ adop phase=apply patches=18283295,20034256,20075842 hotpatch=yes merge=yes

which fails with:

APPL_TOP is set to /ora12apps/ebstest/fs1/EBSapps/appl

[UNEXPECTED]adop cannot detect any application tier nodes in FND_NODES table. Ensure ICM is running and run autoconfig on all nodes

[UNEXPECTED]Error while checking if this is a multi node instance

Log file: /ora12apps/ebstest/fs_ne/EBSapps/log/adop/adop_20150105_080338.log

adop exiting with status = 2 (Fail)

Anyone have any ideas?  I have searched for hours, but cannot find a document with a solution that works.
