12.1.3 - PO Approval Notification Error at Generate PDF in Buyer's Lang

12.1.3

I have been looking around KB and Community, and I have not found an exact hit for this error and/or the solution was not defined.  PO Issued, started the approval process, it made it through 2 approvals and then stopped.  It says it made it to the last approver via the Action History, but he has not received the notification.    The failed activity is below.  We are not having issues with other PO's, just this one....currently. 

I tried rewinding the WF activity to the active step, result is the same error.  Do we need to do the work around of moving the PO from In Process to Re-Approval to get things jump started again?
