Parameter Data Entry Field Disappears After an additional Parameter

All:

I needed about seven (7) parameters for my BI Publisher Report. I've created 5 of those parameters and it looked fine, but when I added the sixth parameter, the data entry field of the current and the previous ones all disappeared leaving only the labels on the report. Can anyone explain to me why this should happen?
