Error ORA-01406 during wip completion

I am getting fetch column value was truncated on work order-less completion wip completion using a schedule number.  This does not happen all the time seems like about 1/2 the time.

Details of the error message are FRM-40735:  ON-COMMIT trigger raised unhandled exception ORA-06502.
