PO Approval hirearchy Step by Step Guide

Hi,

I will appreciate if some one please provide step by step guide to build the approval hierarchy for PO as i have Three  Users Say TEST_MANAGER, TEST_SUPERVISOR and TEST_USER and i want to send approval through  Supervisor to MANAGER and need step by Step Guide and forms that are involved in it.
