How to Access Smart form in a store through a hyperlink from External System

Hi,

   My Customer using an external system, where on search they will get in which user will directed to used a smart from from a store.

   Now customer needs a hyperlink, in their system. when they click on the link, it should take them direct to the smart form available in I-Procurement store. Is their any way that we can achieve this. looking for input.
