Upgrade Oracle 11203 currently residing on RHEL4 to Oracle 12c

We have a RAC cluster running Oracle 11.2.0.3 DB and ASM and currently residing on RHEL 4.0. We would like to upgrade the Oracle 11g to Oracle 12c. We would like to the recommended process to follow.
