Service - Case Management, Cases tab not showing in Agent dashboard

Hi,

We have installed Oracle EBS 12.1.3 vision instance, and facing following problems in 'Agent Dashboard' screen of 'Case Manager' responsibility---

1. Cases Tab not showing in Agent Dashboard form after search party. However, some times it shows.

2. While pressing the button 'Create Case' after searching party error showing "Unable to find an object with the name - CategorySegmentVO."

Can anyone guide me as what mistake I am doing or is there any setup part that I am missing
