While applying 12.1.3 upgrade patch 9239090 workers fails

Hi

I am applying "9239090" upgrade patch but the patch 9239090 stops with 16 workers failed.

There are now 21843 jobs remaining (current phase=A2):

    0 running, 74 ready to run and 21769 waiting.
