One requisition line feeding into two or more purchase orders; has someone else come across it?

We've a single instance of one requisition line having a stock item feeding into two purchase orders. I believe it should not be possible for the sourcing engine to select an already sourced requisition line and source it to another purchase order.

Both the POs were created at least two months apart and while first PO has already been dispatched. The requisition in question is a replenishment requisition and its status has been Approved when requisition selection and PO creation processes were run approx two months apart.

I'm asking this community if anyone else has come across such a scenario? I will need to carry out a deeper investigation if this is not a legitimate scenario.
