How to Change Form title from Create Service Request Form

Hi,

Please find attached screenshot. Displaying wrong Organization name as Form title. Could you please help me to get navigation to change configuration of form title. Profile options I have verified and it is showing correct.

Navigation: Field Service Manager > Service Request >  Create Service Requests
