adop prepare "SESSION ABORTED" and Middle tiers "ABANDONED"

We have installed new 12.2.4 instance

 

We have 3 tiers:

io190e - DB + Concurrent Tier (Admin Node)

rad004e - MId tier 1

rad003e - Mid tier 2

 

After the installation is completed and the shared appl top was setup, the instance was working fine.

Then we wanted to apply few patches and started prepare phase from admin node.

 

After adop is completed , the status of PREPARE on admin node (io190e) is COMPLETED

and on other 2 nodes , it is NOT COMPLETED (although adop prepare completes successfully next time)

(The issue with 2 mid tiers was with entries in /etc/hosts which was fixed but the prepare phase never completed for those 2 nodes successfully).
