EM showing incorrect Oracle Home for instance

I recently upgraded a database from 11.2.03 to 12.1.02 and I am confused at what steps I need to take in order for EM to now reflect my new Oracle_home for this instance.  I had removed and readded my listener (which I also updated to 12c) and I removed and readded my instance but when I look at the target information for this instance it is still showing the old "Oracle_Home".  It does reflect the version correctly.  Do I need to remove and readd the "Oracle_Home" as well?  Or can I edit or run some command to get this straightened
