Does any one now the solution to this error "racina: ORA-00001: unique constraint (AR.HZ_PARTY_SITES

The customer interface is failing in 11.5.10 with the following error "racina: ORA-00001: unique constraint (AR.HZ_PARTY_SITES_U2) violated".  Is this a sequence issue or something else?
