orachk 12.1.0.2.3 report upload error

This is somewhat odd.  I had previously set up and was running orachk 12.1.0.2.1 and uploading into the APEX Collection Manager and the upload was working fine.  The HTML portion was also uploaded so there was a "View" link that we could click on to see "Actual Values" of issues that were found.  After upgrading to orachk 12.1.0.2.3 the HTML portion does not seem to be uploading so my users are complaining about the loss of the "View" functionality.

 

 

I did not see anything in the notes or user guide about database changes or having to upgrade my collection manager so I'm at a loss as to why this is happening.
