How is the element manipulated when there are two element-rule elements?

If there are two element-rule elements in a header-rule, both are manipulating the same parameter-name, but with different rule, how is the element manipulated? The first element-rule will be applied, and then the second one. Or just the first one that matches the value type? Same for the header rule. (For example, the TO-1 and TO-2 element.
