E1 LDAP User Creation

We upgraded to App 9.1 Tools 9.1 about two years ago.  At that time we enabled LDAP authentication.  It has been a very hands off configuration.  I have a new user that is trying to login but is unable in spite of her account being in the correct AD group and her using the correct credentials. The kickback message is stating 'Incorrect User ID or Password'.  It is my understanding that once E1 authenticates a new user, it will create an account based on the '_LDAPDEFLT' user but this does not seem to be occurring.  Does anyone have any troubleshooting tips?
