EM 12.1.0.4.3 - Seeing error when patching GRID: preRACPatchApplyshell

Using EM 12.1.0.4.3 (PSU3), trying to patch 4 node RAC GRID and seeing the following error in the deployment procedure:

Job Steps Log for Job :Build target-patch specific prerequisite checks

1:Evaluate expression:evaluateExpression

PREREQ_NAME: Check and build the procedure to run the prerequsite.

PREREQ_DESC: Builds the deployment procedure to run the prerequsite checks required by OPlan.

PREREQ_TYPE: TARGET_STATE

PREREQ_STATUS: FAILED

PREREQ_MESG: Unsupported Action Type:oracle.opatch.system.automation.jaxb.CommandAction subtype:preRACPatchApplyshell

PREREQ_REMEDY: MANUAL.

PREREQ_REMEDY_DETAILS: Check the error message and OMS logs for the failure and retry again.

ExpressionEvaluationException: Below error message was returned while executing this Computational step.
