Install base instances creating duplicate assets in Fixed assets

We have an issue in hand where we see that one serial number instance in install base has created duplicate assets in FA. This only happens or happened for some serial numbers and not for all the serial number instances in install base.

The process which we follow is as below-

1.We receive the serial numbers from vendors by PO receipt

2.The receiving TRX creates Install base item instances. It only created one IB instance.

3.We interface these trx to fixed assets running the standard interface program and then run 'Post Mass addition' which creates the fixed asset from the mass addition queue.For some reason it interfaced two mass addition ids to FA.
