12.1.1 Vision upgrade to 12.1.3 -- Prerequisite patch 9239089 error

I am upgrading my vision EBS from R12.1.1 to R12.1.3.

While applying pre-requisite patch 9239089 ( p9239089_R12.AD.B_R12_LINUX.zip )  getting following  re-link  error and files are missing in $AD_TOP/bin  and adpatch is prompting me to " Continue as if it were successful [No]: ".  I followed README.txt  correctly and everything came successfully ( adgrants.pl  & perl $AD_TOP/bin/admsi.pl -cli ) but adpatch is failed due to the following error.

I checked these missing  files before applying this patch, all are present and after applying 9239089 patch files are missing.  Do any one faced this issue earlier. Please check the issue and give me a valuable solution.
