Starting 12c OEM agent - Registry Manager failed at Startup

OEM : 12.1.0.4

12c Agent is failing to start and below are the errors in log and out file. Please advise if anyone has a fix for it.

Starting agent ................ failed.

Registry Manager failed at Startup

Consult emctl.log and emagent.nohup in: /u01/app/oracle/product/agent12cr4/agent_inst/sysman/log
