Bug when adding a Direct Item to a Work Order through Requisition Form

Hi Gurus,

Have any of you encountered any bugs when adding a Description Direct Item to a Work Order through Forms. When I open the WO in OAF Pages, the Item does not show under Direct Items and there is no Requisition either. However, the Item has also been received and WO Costs are also updated.

There is some background to this, prior to this WO, a similar WO for the same job was created previously and a requisition was created for it with the exact same Description for the Description Direct Item. However, this requisition was cancelled.

A few months later a 2nd Work Order (the Work Order under discussion in paragraph 1 of this post) was created. Requisition also created with exact Description Direct Item. No Item showing under Material Requirement, however WO Costs have been updated.
