opmn shutdown unexpectedly oracle forms 11.1.2.2 on windows 2008R2

Hi Guys,

Our environment: Windows 2008R2(64-bit);

Weblogic: 10.3.6.(applied jan cpu 2015);

Oracle forms: 11.1.2.2 (applied January CPU patches);

OEM Agent 12.1.0.4;

All environment running. Once in 1-5 days opmn stopped running unexpectedly.
Before we applied patches, it was not ever possible restart opmn. It was only
one solution reboot server and everything come to normal.

After we applied CPU January 2015 patches: (p18423801_111170_MSWIN-x86-64
and p19948000_111170_MSWIN-x86-64)

it still continue unexpectedly shutdown, but we can restart opmn service
without reboot server.

can anybody help?

What we should do  stopped opmn
unexpectedly shutdown?
