Notifications keep appearing to the same user

Hello Experts

We upgraded to 12.1.3 two weeks ago and now having issues with vacation rules.

One of my users setup a vacation rule to delegate the notification to his assistant before the upgrade, manager has got enough limits to approve the order, when the assistant approved it system is sending the notifcation again to approve, it happened more than 10 times.

Any valuable inputs please?

I have gone through Oracle support but did not come across any appropriate link for this issue
