Depot Repair - Setting up Service types - No Value in LOV

I am trying to create a set of Service types.  We have the Business process set up, the service mode is WIP, the Service Type Ref is Advanced Exchange, and we are not able to select the Service activity - the LOV is empty.

We have validated multiple times that the activities are there, active, set correctly to Return and Order ( with Material billing type). and the Order management Header and Line types are accurate.

This is in my production environment, we have the exact same setups in Stage and they work fine.  I am not aware of any patch differences.
