Why does FDM keeps losing its adapter and how can we prevent this?

We have our system run by OMCS, and have had recurring instances of the FDM adapter disappearing without notice or noticeable cause. Have others encountered this? What can we do to easily restore the adapter? What could cause it to vanish?
