HFM client unexpectedly changing metadata?

Hello,

On both V11.1.2.2 and V11.1.2.3, has anyone else experienced metadata unexpectedly changing when using the HFM client? An example is when making updates to entities in metadata an account may move or the account name may change when the account dimension was not touched.

If you have had this experience, was root cause identified? Were fixes or workarounds identified?
